Including protective factors and collaborating on a safety plan helps balance risk with resources and supports. A clear assessment also centers on the immediacy of danger and whether a higher level of care is needed today, guiding decisions about safety hospitalization or intensive support.

Relying only on current mood or sleep patterns misses active risk signals. Relying on family reports and avoiding direct questions can lead to incomplete or inaccurate information. Documenting a general impression without specific questions provides little to act on, making it harder to determine safety and appropriate care.
